Starting Up: A Step-By-Step Guide

1. Fill out the form on this page! (below on mobile)

2. Have your location approved (Address) (once address is approved, distributor
   will meet with you to discuss color schemes etc.)

3. Sign Dealer Agreement.

4. Discuss your menu.

5. Obtain insurance and list Shaved Ice Enterprises as additional insured for no less than $1,000,000.00, provide Shaved Ice Enterprises with a copy of your Certificate of Insurance.

6.  If you would like your business product purchases from us to be exempt from sales tax, provide Shaved Ice Enterprises with form ST-105.

7. If you are not already a food vendor, contact Health Dept. for a list of requirements and schedule your inspection. See FAQs for lists of local health departments.

8. Pick up and pay for your initial order.

9. Mix and fill all gallon and quart bottles and prepare any other product. Set up your store.

10. Schedule your training session with up to 5 owners/ employees.
